We are now accepting rebate information for 2nd quarter 2008 through October 31, 2008.   Please check back with us for additional updates.

If you have forgotten your password, you can now enter your NCPDP Provider ID, and click on Password Reminder.  You will receive an email with your password very shortly thereafter.  If you do NOT receive an email, please call our help desk to provide your correct email address and have your password reset.
LTC Providers who need to sign the attestation  
To sign the attestation regarding the 5% threshold, you must sign into the website using your NCPDP provider ID and your password.  After logging in, you will be given an opportunity to sign the attestation.

LTC Rebate File Consolidator Information

When :  File submission and file creation for the reporting period 2Q08 is open.  


Participating PBMs/PDP Processors:  Argus, CVS/Caremark (including Pharmacare/Coventry), Express Scripts, FLRx, Medco,  MedImpact, Prime Therapeutics, Rx America, Wellpoint.   If you need to send rebate data to any one of these PBMs, you need only send it once to Premier.  Your data will be distributed automatically to those PBMs with whom YOUR Pharmacy/LTC Facility is contracted.

For assistance with the data content of your submission, you should call a PBM with whom you are contracted:

Submission Methods:  A web-based FTP server is available if you will create your files on your own and send them to us. A link to the FTP server is in the Helpful Links section on this home page. If you prefer to create your rebate file using our web-based entry method, sign in to this site with your user name and password, and you will be taken to the rebate submission page. 

Pre-Formatted EXCEL Document:  For pharmacies choosing to enter their data in a file and transmit that file to the FTP Server, you may click on the link below to download an EXCEL spreadsheet already formatted with the column headers and numeric formatting necessary for the Rebate $ Amount Field.

Web-Based Data Entry:  After you log-in, you will be taken to the page where you will make your entries for the rebate file.   You will need to select the year and quarter that your rebate file applies to, then begin making your entries.
